Fully accomplished Personal Data Sheet (PDS) with a recent passport-sized picture (CS Form No. 212, Revised 2017, which can be downloaded (here);Performance rating in the last rating period (if applicable);Eligibility or Board Rating (applicants for positions that do not require practice of profession), or Valid License (applicants for positions that require practice of profession;For Aircraft Mechanic applicants, aside from the CAAP license, submit an authenticated copy of the Knowledge Test Report;Authenticated copy of Transcript of Records/Diploma;Birth Certificate/Marriage Contract (PSA); andComplete Local Clearances (Barangay Clearance, Mayor’s Clearance, Police Clearance, RTC Court Clearance, NBI Clearance, and D.I. PNP Clearance)How to Apply
